Access Pointe Foundation closes the gap between the care people need and what their insurance, income, or circumstances allow — so no one has to choose between their health and their home.
We work at the access point: the exact moment someone is told “not covered,” “not eligible,” or “not enough” — and we build the bridge across it.
We start with the people most often left behind by the systems meant to protect them: seniors facing complex medical needs, adults with disabilities aging out of parental care, and veterans without a safe place to land.

Each program answers a different version of the same question: what happens when the people who love you can no longer be the whole safety net?
01—Seniors
Harbor for Seniors
Because a healed wound shouldn't cost you your groceries.
Harbor for Seniors provides medical supplies, equipment, and care access for elderly patients whose insurance coverage—often Medicare Advantage—will not cover the advanced treatment they need. This is the program Access Pointe Foundation was founded on: helping patients get the care that saves their limbs and their independence, without forcing an impossible trade-off with their basic needs.

Because every child deserves a safe place to grow.
Harbor for Foster Care supports children and youth in foster care by helping connect them with essential resources, stable living environments, and the support they need to thrive. The program aims to help bridge gaps in care, improve access to basic necessities, and support smoother transitions toward adulthood and independent living.
03—Veterans
HARbor for Veterans
They served their country. We make sure they have somewhere to come home to.
Harbor for Heroes provides housing support and care access for veterans, addressing the gap between the service they gave and the stability they're often denied on return. This program extends the Access Pointe model—meeting people at the exact point the system stops covering them—to the veterans our country too often leaves to navigate that gap alone.
04—Neurodivergent
HARbor for Neurodivergent Community
Because being different should never mean being left behind.
Harbor for the Neurodivergent Community supports neurodivergent individuals and their families by helping improve access to essential resources, appropriate care, and supportive environments. The program aims to bridge gaps in services and provide practical support that respects each person's individuality, strengths, needs, and choices—helping individuals build meaningful, fulfilling lives with dignity and greater independence.

Access Pointe Foundation has helped patients switch insurance coverage to access advanced wound care they were being denied — closing wounds, preventing amputations, and helping people remain independent in their own homes.
